Application Checklist

š   Pay stubs for 2 most recent pay periods
š W-2 statements for the previous year
š ALL PAGES of Bank and Brokerage (Asset) account statements for the
     most recent month
š Copy of an acceptable photo identification (current and valid driver’s license,
     U.S. passport, military ID, etc.)

Also, if applicable, Borrowers may need to submit:
š Two most recent years tax returns for the following income as well as documentation
      to indicate that this type of income will be ongoing:
š Overtime income
š Bonus income
š Child support
š Income from partnerships
š Income from rental properties
š Rental, lease, and/or partnership agreements
š Fully executed, final divorce decree
š Bankruptcy petition and discharge papers
š Written explanations of any of the following:
š Late payments
š Bankruptcy
š Defaults and foreclosures
š Judgments and liens

For Self Employed Borrowers:
š Two most recent years complete tax returns, including all schedules and pages
š Copy of business license or letter from CPA verifying 2 years of business’ existence

For Purchase Transactions:
š Copy of the purchase (Earnest Money) contract
š Gift letter (if gift funds are being received from a direct family member) that
      includes amount, relationship of donor, verification of gift source and confirmation
      that no repayment is required (if applicable)
š Final settlement or closing statement from sale of present property
      (if applicable and when available)
š Name and telephone number of homeowner’s insurance agent

For Refinance Transactions:
š Survey, if applicable in your state
š Current homeowner's insurance policy “Declaration” page


For Condominiums and Most Attached Townhomes:
š The contact name and phone number for the Home Owners Association
š Have the Association provide a copy of the master insurance liability and
     fidelity bond declaration page to us.
